March is Breast Implant Awareness Month, an effort dedicated to ensuring women who have breast implants – and those who are considering breast augmentation – have all of the information they need to make informed decisions about their health. While breast augmentation is incredibly popular and considered safe for most patients, complications can arise. Understanding these potential complications along with treatment options, such as breast implant removal and revision, can be useful when weighing the pros and cons of breast enhancement.

While breast implant complications are uncommon, they are not impossible. You can help to reduce your risks of developing a problem by closely following pre and postoperative directions and bringing any concerns to the immediate attention of our office.

Implant Removal & Revision

Breast implant removal may become necessary due to complications with scar tissue, changes in implant appearance, implant rupture, or implant shifting. These issues may result in discomfort along with changes to the appearance of the breasts. They can often be addressed through revision surgery.

During revision surgery, older implants are removed and replaced with new ones. The new implants can be of a different size or shape, allowing for complete customization and helping to produce the most desirable results.

Some women choose to undergo implant removal alone. When this is done, the breasts simply return to the size they were before augmentation. If sagging is present after the removal of implants, breast lift surgery can be used to restore a perky and lifted appearance to the chest.

Incision Markings on a Female Breast

There are three primary breast lift incision options. The one that is best for your needs will be dependent on factors unique to your breasts, including their size and the amount of ptosis (sagging) that is present.

The three most common beast lift incisions are:

  • Concentric. Also known as the “donut” incision, this is done in a circular pattern around the areola and is ideal for mild ptosis
  • Circumvertical. Commonly called the “lollipop” incision, this option goes around the nipple with an additional incision from the bottom of the nipple to the breast crease. It is best for moderate ptosis
  • Inverted T. For more pronounced ptosis, an “anchor” incision is best. This option includes the incisions from the lollipop technique with an additional incision following the breast crease

With each incision, great care is taken to help guard against highly visible scarring. You can help to reduce your risks of thick and noticeable scars by keeping incisions dry and clean, avoiding blood thinners and vasoconstrictors during the recovery process, and following the advice provided by our office before and after your procedure.

Anyone considering a breast augmentation or breast lift (mastopexy) with implants to improve their breast shape/size is routinely confronted with “plastic surgery gone wrong” horror stories.  These myths almost always make the decision to proceed with a breast modifying procedure more complex.  So what are the most common myths, and what do you really need to know?

1.) You need breast implants for a lift – FALSE. Only a breast lift, or mastopexy, can correct redundant skin and sagginess of the breast.

2.) You cannot breast feed after a breast augmentation – FALSE.  Breast augmentation is typically performed with an incision through the inframammary fold, arm pit, or belly button and should not result in a problem with breast feeding.

3.) Natural breast augmentation with fat is better than breast implants – FALSE.  Transferring a patient’s own fat to the breast after liposuction is a new procedure in the US, currently without regulatory approval, and thus no standardized practice amongst surgeons. The long-term safety considerations have yet to be fully studied, making this a unique and controversial procedure. Additionally, not all of the fat will survive the transfer, making future problems with symmetry and a pleasing aesthetic appearance difficult to forecast.

4.) Under the muscle is more natural looking than over the muscle – DEPENDS.  In breast augmentation, the decision to place the implant under the pectoralis major muscle or subglandularly over the muscle is entirely dependent upon a patient’s individual anatomy.  Differently, patients will achieve different results depending on their own anatomy.  This decision should be made between the patient and the surgeon during the consultation.

5.) Saline is safer than silicone – FALSE.  Saline implants gained popularity because silicone implants were thought to cause medical problems.  After nearly twenty years of clinical study, this has not been proven true.  In fact, silicone breast implants have been proven to perform equally over their lifespan as saline breast implants.  Silicone implants are generally regarded to provide a more natural feeling than saline implants; however, saline implants require less surveillance and do not require MRIs.

Cosmetic surgery: When is the right time?

When considering a cosmetic procedure, whether it is Botox injections or a breast augmentation, you may ask yourself, “Is this the best time to have it done?”  Deciding to have plastic surgery is a personal decision, of course, and timing depends at lot upon your goals and desired outcome.  Below are a few guidelines to keep in mind.

 

Plan Ahead

If you want to be ready for a special occasion, definitely plan ahead.  Many non-surgical procedures such as Botox®, Dysport®, and dermal fillers (Juvederm®, Restylane®, Perlane®, Sculptra®) have immediate results and require little to no downtime.  You may want to look your best for a wedding, family party, or class reunion, but you should give yourself about 2 weeks for any minimal bruising to resolve.

Most surgical procedures, such as a tummy tuck, liposuction, and breast augmentation, do require several weeks to fully recover.  So if you want to look your best in your bathing suit or for that special event, you should plan to give yourself time for the swelling to resolve and for your incisions to heal.

 

General Guidelines for Popular Procedures

 Tummy Tuck (Abdominoplasty):  Maybe your abdomen protrudes after having children, or you have excess skin or bulging at your waistline you can pinch.  You exercise and watch your diet, but your shape is just not what you want it to be.   A tummy tuck may be helpful by removing excessive skin and fat, repairing the abdominal muscles separated by pregnancy, and even improving a C-section scar.  In most circumstances it is best to wait until after you lose excess weight and have finished having children.  Give yourself about 12 weeks to heal before your bathing suit debut or your special event.

Breast Augmentation:  Generally speaking, there are two time periods when a woman seeks breast enhancement surgery.  A young woman in her twenties may recognize that she has finished developing breast tissue and she desires larger breasts.  A breast augmentation with saline or silicone gel implants would provide the best option to help her achieve fuller breasts.  An older woman, later in her thirties, forties, or fifties, may have lost volume after having children, or may notice sagging of her breasts.  A breast lift, or mastopexy, can remove excess sagging skin and can reposition the nipple on the breast.  Depending on your needs, Dr. Doyle will recommend one procedure or a combination of a breast augmentation and breast lift to help you achieve your goals for breast enhancement.  Recovery from breast augmentation is usually quick, but do give yourself a few months before a major event to look your best.

Liposuction:  Liposuction can be used to treat stubborn fat pockets in many parts of the body including the abdomen, thighs, hips, buttocks, neck, back, and chest.  Liposuction is not a treatment for obesity and is not an effective treatment for cellulite, the dimpled skin that sometimes appears on the thighs, hips, and buttocks.  Removing excess fat deposits improves your body contour and proportion, and ultimately can enhance your self-image.  Winter is a great time to do liposuction since post-operative support garments can be easily hidden under clothing.
